Waiver of Liability

For valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the undersigned (and parent/guardian if applicable) acknowledge(s) that there are inherent risks and dangers involved in the activity of BASKETBALL, accept(s) full responsibility for any and all claims, damages or injuries of any kind that may result from this activity and release(s) and forever discharge(s) the Philippine Basketball Association of Ottawa (PBAO), all its directors, officers, organizers of this tournament, sponsors, coaches, trainers, advisers and agents including the host team City from all claims, damages, actions or causes of action howsoever arising whether through negligence or otherwise, as a result of my (my child's) participation in PBAO's practices and out-of-town invitational tournament wherever situated.

I (we) do hereby covenant and agree to indemnify and save harmless all the organizers of PBAO, its directors and officers, organizers, coaches, trainers, advisers from all claims, damages, actions or demands which may hereafter be brought against them by me (us), or on my (our) behalf.

Do I need to pay online?

No. Online payment is not currently required. After your registration is reviewed, you'll receive payment instructions by email.

What is the registration fee?

The 2026–27 season fee is $350.00 per player.

What is the refund policy?

A partial refund of $300 is available if a player withdraws after the first two practice sessions. No refund is available after that point.

What happens after I submit the form?

You'll receive a confirmation, and the program administrator will follow up with next steps, including payment and season details.
